with the support of Senior Millage funding, provides
services to the senior citizens of this county. Staff
participate in community coalitions directed toward
the needs of senior citizens. |

Description of Services
- Provision of flu clinics
- Educate seniors on relevant health topics
- Participation in annual Senior Fairs
- Referrals and information resources for geriatric
services
- Hypertension clinics
- Tetanus-Diphtheria (Td) booster shot clinics
- Health screenings
- Dementia presentations
Home visits to seniors to provide:
- Assistance with independent living problems
- Home geriatric nursing assessments
- Assistance in problem resolution
(i.e., clarification of medication regime)
- Assistance with the establishment of a support
system
- Referrals to appropriate services and agencies

No charge for home visits or consultations.
