Early Periodic Screening, Diagnosis and Treatment (EPSDT)
provides health and developmental screening and referrals
for individuals from birth to age 21 who are currently
covered by Medicaid.

Physical examinations include:
- Immunizations
- Hearing screening
- Vision screening
- Lead screening
- Hemoglobin
- Denver Developmental testing

Eligibility for Services
All individuals under the age of 21, who have Medicaid,
are eligible to receive services.
Cost of Services
Services are provided free of charge to individuals
eligible for the Program.
Hours of Service
Appointments can be made Monday - Friday, 8:00 a.m. -
12:00 Noon, and 12:30 p.m. - 4:00 p.m. Appointments
are required to receive services.
